موارد مصرف اُگزالیپلاتین:
Oxaliplatin یک ترکیب حاوی پلاتینوم است مشابه با سیس پلاتین در درمان سرطان کولورکتال متاستاتیک این دارو همراه با فلورواوراسیل و اسید فولینیک تجویز می گردد. دوز توصیه شده mg/m 85 به طریق انفوزیون داخل وریدی طی 2 تا 6 ساعت می باشد که در 250 تا 500 ml گلوکز 5% حل می گردد. دوز تجویزی را می توان در فواصل 2 هفته ای تکرار نمود. جهت کاهش عوارض تولیدکنندگان پیشنهاد می کنند دوز آغازین را به mg/m 65 کاهش دهند. همیشه قبل از fluoropyrimidine ها باید Oxaliplatin را تجویز نمود. جهت درمان سرطان تخمدان نقش این دارو در دست بررسی است.
فارماکوکینتیک اُگزالیپلاتین:
متعاقب دوز داخل وریدی این دارو به صورت وسیع در کل بدن توزیع می گردد و به صورت برگشت ناپذیر به گلبولهای قرمز خون متصل می شود که این باعث طولانی شدن نیمه عمر دارو می شود. نیمه عمر نهایی متوسط دارو بین 373 ساعت و 391 ساعت متغیر است. این دارو به صورت وسیع به ترکیبات غیر فعال و فعال متابولیزه می گردد و عمدتاً از طریق ادرار دفع می گردد.
عوارض جانبی اُگزالیپلاتین:
این عوارض مشابه با عوارض ناشی از سیس پلاتین می باشند اما حالت تهوع و استفراغ و نفروتوکسیستی و مهار شدن میلوئید کمتر واضح می باشند. افزایش مقادیر آنزیم های کبدی می تواند رخ دهد. مسمومیت عصبی باعث محدودیت دوز می شود و در 85 تا 95% بیماران نوروپاتی محیطی رخ می دهد و درد و اختلالات عملکردی و از بین رفتن رفلکس های تاندونی می توانند ایجاد شود. فیبروز ریوی که می تواند کشنده باشد نیز مشاهده شده است. خارج شدن Oxaliplatin از عروق می تواند باعث ایجاد درد موضعی و التهاب شود و این عوارض گاهاً شدید می باشند و باعث ایجاد نکروز می شوند.
معاینه عصبی باید در فواصل زمانی منظم در طی درمان انجام شود و در صورت طولانی شدن علایم و شدت یافتن آن ها دوز تجویزی باید کاهش یابد. شمارش منظم گلبولهای خون باید در طی درمان صورت پذیرندو تا بهبودی شمارش آن ها نباید اقدام به تکرار دوره درمانی نمود. در بیماران مبتلا به نوروپاتی حسی از قبل و یا مهار بودن میلوئید و یا مبتلا به اختلالات شدید کلیوی نباید این دارو را تجویز نمود. در آنهایی که دارای درجاتی متوسط از اختلالات کلیوی هستند عملکرد کلیوی و مسمومیت باید به طور دقیق بررسی شوند.
